STUDENT PHOTOGRAPHS, VIDEOS, AND/OR SOUND RECORDINGS OPT-OUT

A parent may withhold permission to have a student photographed, videotaped, and or audiotaped during studio-sponsored activities, learning experiences, and/or media events.

As the parent or guardian of the student identified above, I understand that if I opt-out, my child’s lessons will not be included in our teacher coaching and mentoring program, or shared with anyone outside the school, including commercial photographers and the media, nor will my child’s picture be part of studio programs, events, or any other medium.

Note: This does not include videotaping by security cameras in school.

DO NOT allow my child to be photographed, videotaped and/or audio taped during their private lessons. I understand that my child’s image may be recorded if they choose to participate in studio events such as performance parties, Saturday Song Shares and more. Their image will not be used for marketing purposes regardless. They may join those events and keep their camera off or simply choose to not attend.
